Insurance
We participate with many different PPO and HMO plans and accept traditional indemnity insurance. If you are not sure about our providers’ status with your insurance company, please check with your insurance company or your employer’s benefit representative. Most cosmetic procedures are not covered by health insurance plans.
Patient Financial Policy
We are dedicated to providing the best possible care and service to you and want to insure your complete understanding of our financial policies. We have contracted with many health insurers and health plans to accept assignment of benefits. If we have a contract with your insurance company, you will be required to pay only the co-pay when you arrive for your appointment.
Some insurance plans require referrals to specialists (Dermatologists) and refuse to pay your charges if one is not obtained. Therefore, you must check with your insurance plan.
If a referral is required from your primary care physician, call your primary care physician prior to your appointment with us. If the referral has not been received by our office when you arrive, the visit may be rescheduled or you may be responsible for payment.
All health plans are not the same and do not cover the same services. In the event your health plan determines a service to be “not covered,” you will be responsible for all charges. Payment is due upon receipt of a statement from our office.
Most cosmetic procedures are not covered by insurance and payment will be the responsibility of the patient.
